5.05.120 License application – Contents.

Every person required to have a license under the provisions of this chapter shall complete the required licensing application and submit it to the secretary of the tribe or his/her designee at the tribal administration office. Such application shall be in writing and shall contain the following:
(1) The name and address of applicant;
(2) The name of the business to be licensed;
(3) The type of business activity to be licensed;
(4) The place where such business is to be transacted and carried on;
(5) The date of the expiration of such license;
(6) Such other information as may be necessary for the enforcement of the provisions of this chapter. [Ord. 05-31-2018-A § 2(4.01.120), 2018.]
